Welcome to the Automation Project Intake form! What follows will allow us to obtain the required information in order to start setup of your new account.

This form will take about 30 minutes to complete.

What name and email address should emails be sent FROM?(Required)

IE: Lindsay Kirsch ([email protected]). This must be a domain email address (we cannot use .gmail, etc.) If a domain email address does not exist, it must be created by the web team first.

What should the sender signature include?(Required)

Example: headshot, name, title, company, phone number

Max. file size: 128 MB.

What email address should responses to your emails be sent TO?(Required)

It may be the same as the sender, or different. This email will get all responses include contact out-of-office and other automatic responses.

What 2-3 CTAs would you like to have in the header of each of your emails?(Required)

This should be a brand-specific CTA such as “Shop Now” or “View Services” and usually matches your most prominent website CTA. Please include links to the pages on your website. Example: https://share.zight.com/E0uekpYG

What 2-3 CTAs would you like to have in the footer of each of your emails?(Required)

These are usually the same CTAs as used on the footer of your website. Please include links to the pages on your website. Example: https://share.zight.com/lluWqL5g

Would you like to include your company’s support number and availability in the footer of your email? If so, enter below:(Required)

Do you have any preferred contact segments (how you want to be able to sort and view contacts?)

It is required that all emails have an unsubscribe link. In addition to this, we recommend setting up email preferences. Do you agree with this user preference text to be placed at the bottom of every email?

ACTIVECAMPAIGN ACCOUNT

What is your ActiveCampaign account URL?(Required)

Please create a CFMedia Admin account associated with the email [email protected] and the phone number 904-208-2747. Once this is created, we will reset the password to login.(Required)

An additional login will only be able to be created once you are on a paid plan.

Please list the names and emails of any other additional users that you would like to be setup on the account. (If you are not sure, we can update this later).(Required)

Note: additional users may require additional ActiveCampaign usage fees.

Who should be the default Deal Owner for CRM Deals?(Required)

We can always create additional deal owners later on. This is for the initial setup.

Intergrations

Does CFMedia have website access available in 1Password?(Required)

Do you want to have emails that invite users to schedule a consultation with you or your team?(Required)

If yes, please provide the scheduling instructions or link.(Required)

If yes, do you want to integrate your ActiveCampaign account with Calendly for appointment scheduling?(Required)

Contact Migration

Are we going to migrate contacts from another system over to ActiveCampaign?(Required)

Provide a link to access to the .CSV file of contacts or email access to [email protected].(Required)

Note: we will not accepted emailed list files as they contain PII (personally identifiable information) and it is not secure. Alternatively, you may provide CFM admin access to your current system and we will export the list data.
